Now, this is not strictly because of physical exhaustion, but largely because of lack of sleep, persistent training even when the body is screaming for rest, and playing with unstructured or structurally imbalanced bodies. They persist with it and end up breaking apart the whole system.
Then, regardless of how big your butt is, your glutes are not firing as they should, making you slower and more injury prone. In addition to this, wrong mechanics, whether in running, jumping, or repetitive actions in the case of fast bowling, throw the whole balance out of the window, creating inhibited and overactivated muscle groups.
It plays out as I have explained above. Give your body what it requires. Understand which systems are taking more stress due to persistent load, training, spikes, or anything else. Do something about it. We cannot take an imbalanced structure and expect it to keep performing at its best. The body handles it initially because it has resilience, but just like aging, it gives up one thing at a time.

a) Yes, anybody slowing down, not improving, or feeling fatigued tells me that there is an issue. Now, spotting that issue is an art that comes with expertise. The clue is always visible in them.
b) HUGE. I think my phenomenal success in the last camp was because of a strong mind component. We did a one hour session every day for relaxation, recovery, and mindfulness. Since they are athletes, I kept these sessions as a mix of movement, strength, stillness, balance, fascia opening, and the required stretching depending on the damage that their sport had accrued due to repetition, eventually leading into mindfulness.

No, the load is common. Their bodies make it harder or easier.
If the effort is meant to be hard, then fitter players will make it hard by putting in more effort, speed, and striving for 100% perfection, whereas the weaker players will keep improving with every session. They may have different thresholds for everything, and therefore the training implication is such that those who can put in greater effort will naturally do so, while those who are still getting there will continue to improve.
The key, again, lies in my one line solution: recovery. It is crucial for both. It is my hypothesis that a good recovery session helps both groups restore their equilibrium. The next day, they are back again with the same energy.

It is universal in the end. Some players adapt to things faster than others, but that is the whole idea of a recovery cum relaxation session: to bring each individual back to their balance.
I think if the training foundation is such that, instead of competition, it allows each player to contribute their best with intense focus, then improvement is obvious.
In my studies, it is amazing how consistent the improvements are across the subject group. Those who improved their running time by 11 seconds included both good sprinters and average sprinters. Their improvement was remarkably similar. The same applies to fast bowlers. Those who improved the most included slow, medium pace, and fast bowlers, right across the spectrum.
